A Community Partnership Supporting Senior Independence
Thryve provides seniors with access to the support, resources, and programs needed to maintain independence and wellbeing at home. Through a network of local partnerships, this community-based initiative helps older adults age with confidence and purpose by staying healthy, happy, and connected wherever they are most comfortable.

With a combination of in-person programs, community partnerships, and at-home services, Thryve creates pathways for older adults to stay connected to one another and to the support systems they rely on most.
Thryve serves individuals who want proactive, adaptable ways to support their wellbeing without committing to a higher level of care. Whether they’re living independently or transitioning through different stages of aging, our programming is designed to meet them where they are—socially, emotionally, physically, and spiritually.
Bridging Community and Care
In November 2024,
Guardian Angels Senior Services was awarded a $600,000 grant to help address barriers facing older adults in rural Minnesota. Residents in these areas face greater challenges in navigating access to support and resources for maintaining overall wellbeing as they age. This, in turn, makes it more difficult for them to remain in their homes and live independently.
Powered by Guardian Angels and
LeadingAge Minnesota, Thryve is a collaborative effort between local organizations, service providers, and trusted partners. It expands access to the services and opportunities seniors need for living well through social gatherings, educational sessions, intergenerational programs, and technology-based connection.
Connected Communities
From social isolation and fragmented services to difficulty accessing the right resources, seniors often face challenges that affect physical, mental, and social well-being. As a result, the Connected Communities project has transformed into Thryve, a community-powered initiative dedicated to helping seniors strengthen their health, build meaningful connections, and easily navigate local supports.
Together with local partners, Thryve is creating an integrated network that empowers older adults to live well, stay independent, and age successfully where they feel most comfortable.
With support from our partners, Thryve helps make local services easier to navigate while strengthening the connections between health providers, social services, and community resources. By working together, we’re closing critical gaps and ensuring that seniors who live independently have access to the right support at the right time.
This collaborative approach helps older adults stay well, stay connected, and Thryve at home in the communities they love.
